When you visit the doctor, you are looking for an expert who will care for your medical needs. But we know that there many other needs that affect our health. These include such things as:

  • Food insecurity
  • Utilities
  • Clothing
  • Transportation accessibility
  • Employment and employment readiness
  • Access to childcare
  • and more…

These non-medical, health-related needs are called Social Determinants of Health (SDOH). BCHC’s SDOH initiative begins with a short survey all of our patients are asked to complete prior to their appointment. Our SDOH team then assesses your needs and works with your primary care provider and key staff to address these needs and provide resources. As we move through this process, the goal is to build a trust-centered relationship that moves beyond the basics of medicine and into a focus on your overall health.
