The Nurse Care Coordinator RN serves as the leader of a team comprised of a Medical Assistant (MA) and Patient Services Representative (PSR) that support Providers to integrate each patient’s care across the life span. The Nurse Care Coordinator (NCC) will work with the Clinical Manager, Chief Operating Officer, Outreach Manager, BCHC Management Team and others to develop processes and systems that operationalize BCHC’s mission.
